In accordance to the report No. 15/2012 dated April 27th, 2012 the Management Board of Ciech SA hereby forwards the résumés of the members of the Supervisory Board appointed by Extraordinary General Meeting on April 26th, 2012 in accordance to their statements:
Zygmunt Kwiatkowski – Member of the Supervisory Board
A graduate of the University of Maria Curie Skłodowska – The Faculty of Mathematics, Physics and Chemistry (1979). He completed postgraduate studies for bank management staff on the Faculty of Economics at the University of Warsaw (1997). He completed a PhD at the Faculty of Organization and Management, University of Warsaw. He obtained the title of Doctorate Major in Business Administration at University of Nort West Columbia (2004). He completed courses and trainings in financial analysis, management, financial instruments and stock exchange issues.
In 2002-2006 he served as President of Puławy Nitrogen Plants. From 2012-2011 he was Chairman of the Management Board of Wiko Company SA. Currently he serves as Vice-PresidentKruszywa Niemce SA and runs his own business under the name Cooperation and Global Consulting Zygmunt Kwiatkowski.
He was a member on the supervisory boards Puławy Nitrogen Plants (2001-2002), Police Chemical Plants SA (2010), F.S. Holding in Lublin (2002). He is currently Chairman of the Supervisory Board of Z.A.O. A.F.K. in Kiemierow (The Russian Federation) – the construction of the combine of wood processing and sits on the Supervisory Board of P.P.M.B. Niemce S.A.
Mr. Zygmunt Kwiatkowski declares that he does not participate in any rival partnership and is not a member of the other legal entities competing against company, he also does not perform any other activity beyond of the said. He is not listed in the Register of Insolvent Debtors maintained pursuant to the Act from August 20th, 1997 r. on National Court Register (Act of Laws of 2007, No. 168, item 1186).
Maciej Lipiec – Member of the Supervisory Board
A graduate of Cracow University of Economics (2000 r.), graduated from the Małopolska School of Public Administration / University of Copenhagen with the title of Master of Public Administration (MPA), he was a scholarship student at Stockholm University (School of Business).
In the years 2000 - 2002 he worked at Commercial Union PTE Financial Services and Open Pension Fund BPH CU WBK. In the years 2002 - 2003 he was employed in the Ministry of Economy, Labor and Social Policy in the EFS Department. In the years 2003 - 2004 he served as an advisor to Deputy Prime Minister and Minister of Economy and Labor of the Ministry of Labor Economy. In the years 2004 - 2005 he was Director of the Secretariat of the Deputy Prime Minister Chancellery of the Prime Minister. In the years 2005 - 2011 he was employed at the Industrial Development Agency SA. Currently he serves as CEO at Operator ARP Sp. z o.o. He is certified PRINCE2 Project Manager and Risk Manager.
He was a member on the supervisory boards in Krakow Tannery Works SA (2005 – 2006), Operator ARP Sp. z o.o. (2007 – 2011) and Z.E. Elsen SA (from 2011).
